Using pacifiers with breastfed babies always seems to be a controversial topic. Can my baby have a pacifier? Should I wait to introduce one? Will it cause nipple confusion? What are the best pacifiers for breastfed babies? The questions could go on. The American Academy of Pediatrics (AAP) 2022 updated breastfeeding guidelines reports that a review shows introducing a pacifier before or after the mother and baby duo establish breastfeeding does not impact the duration of breastfeeding for a healthy, full-term baby up until four months of age.1

You may think, “This is great news, but is there a catch?” Not really, but here are some guidelines to make sure you and your baby use a pacifier appropriately to not interfere with breastfeeding. Ultimately, the decision is up to you, the parent — pacifier or no pacifier. Let’s dive into the benefits, risks, and guidance for how to pick the best pacifier for breastfed babies.

Benefits of Pacifier Use

It might be hard to believe, but there are benefits to infant pacifier use. Let’s start with one of the most important benefits. Research has shown that when an infant uses a pacifier when placed to sleep, it can decrease the risk of sudden infant death syndrome (SIDS). Pacifier use has also been shown to provide infants comfort, has been associated with helping infants when they’re in pain, and even can help premature infants develop a sucking reflex. And lastly, introducing the pacifier decreases the risk of developing a thumb- or finger-sucking habit.2

Risks of Pacifier Use

You must always weigh the benefits and risks to determine what is best for your baby. This is even true when deciding whether to introduce a pacifier to your breastfed baby. There are a handful of benefits to pacifier use but also risks. One of these risks includes the impact of pacifier use on developing otitis media as an infant and later as a child. Pacifiers also pose an increased risk of causing problems such as oral yeast infections due to carrying bacteria and other germs. And, after 36 months old, the risk of developing dental and orthodontic issues rises.2

Important Note for Pacifier Use for Breastfed Babies

Pacifiers are an excellent tool for helping babies soothe and calm down when upset. It is essential to pay close attention to your infant’s hunger cues. Do not delay or miss a feeding by giving your baby the pacifier. The pacifier should never be a substitute for the breast.

How to Choose a Pacifier for Breastfed Babies

The AAP gives the following guidance on choosing a pacifier:3

  • The pacifier should not be able to come apart. Multiple pieces can increase the risk of choking. The pacifier should be of single construction.
  • The pacifier should have ventilation holes and measure at least 1.5 inches across. This prevents the baby from fitting the whole pacifier in their mouth, helping prevent choking.
  • Do not attach a pacifier to a cord or strap, especially around the infant’s neck. This could lead to an injury.
  • Never use a bottle nipple as a pacifier. The full nipple could easily go into the infant’s mouth.
  • Do not force a baby to use a pacifier if they do not want it.
  • Regularly inspect the pacifier before giving it to the infant. Pacifiers can break and deteriorate.

Although the AAP gives guidance on what to look for in a pacifier, they do not specifically recommend a type or brand of pacifier. The vast pacifier market has many shapes, colors, nipple sizes, and materials. It may be a trial-and-error process to see what pacifier works best for your baby. A breastfeeding baby may benefit from using a pacifier that closely resembles the mother’s nipple, but no research supports this.

Over the years, I’ve been around a lot of breast pumps. I’ve assembled them, sterilized them, used them personally when I had my son, and I’ve helped mothers learn how to use them. Pumping is a common topic that I cover with my clients as a postpartum doula. And having helped hundreds of families with their newborns, I know which pumps are the best breast pumps. But before you go deciding on which is the best breast pump for you, it’s important to understand the different types of breast pumps and know what to look for.

Types of Breast Pumps

There are four different types of breast pumps:

1. Manual breast pumps:

These breast pumps are operated by hand.

  • Pros:
    • They are lightweight, small, portable, and easy to store.
    • They are great because you can use them to pump anytime and anywhere.
    • They are also inexpensive.
  • Cons:
    • With manual breast pumps, you will be doing all of the work. They have a lever pump or bulb suction for you press together and squeeze repeatedly to extract your breastmilk.
    • This can be tiring for you and your hands, so you may not get as much milk as you would with an electric breast pump.

If you plan on pumping regularly for your little one, I wouldn’t recommend this style of a breast pump for you. A manual breast pump is a good option for occasional pumping sessions, especially when on the go.

2. Battery-operated breast pumps:

These breast pumps are similar to electric breast pumps because they do the pumping for you, however, they use a battery that you either need to charge or continue to purchase to operate.

  • Pros:
    • A battery-operated breast pump is great for mothers who need their pump to be portable.
    • They are used and recommended by mothers who have to travel or commute a lot for work.
    • They are great because you don’t have to do the manual pumping yourself and you also don’t have to be tethered to an electrical outlet for pumping sessions.

Closed-System vs. Open-System: What’s the Difference

People don’t normally notice these terms when they first start looking at breast pumps. Once they have decided on the type of breast pump they need, they start looking at the features. These are important terms to know and understand when looking at the features before you make your breast pump selection.

Closed-system pump: A closed-system breast pump has a barrier that does not allow your breast milk to come in contact with the pump’s tubing. This means that your milk goes straight from your breast to the bottle. This is a good thing because it is considered the most hygienic. It can also be very difficult to clean and sterilize the tubes with milk inside them. They are also called “overflow protection.”

Open-system pump: An open-system breast pump doesn’t have a barrier between the milk and the pump. Your breast milk flows through the tubing and other small pieces into the bottle. If the pumping kit and tubing are not cleaned and dried properly it can create an issue. Water or your breast milk can potentially get into the pump and break the motor. Open-system pumps are usually less expensive than closed-system breast pumps.

What to Look For

When researching the best breast pumps, some other things you might want to consider before making your selection are:

Weight:

How much does the pump weigh? If you know that you’ll have to commute or travel a lot for work, you’ll want to consider what you’ll be bringing with you since you’ll have to pump every so many hours. A lightweight pump would be recommended.

Portability:

Is it easy to transport and store away? How many pump parts are there and are they all together bulky? Also, will you need to pump in the car or on a plane, etc.? A battery-operated pump might be the best breast pump for you if you can’t be near a power source. Just be sure to check how long the battery-operated pump lasts on a single charge and if it comes with a rechargeable battery or standard batteries. Another good thing to check is if the pump has a DC power adapter so that it can be charged and used in a vehicle.

Sound:

Are you going to be around other people during your pumping sessions? Do you want to pump discretely? Some breast pump motors are quieter than others so look for a pump that has a quiet motor.

Strength of suction:

Do you already know that you have a slow letdown reflex? Do you need help getting your breast milk to flow? Or maybe need help fully emptying your breasts? Do you want your pumping sessions to be faster? If so, you should look for a pump with stronger suction.

Customization:

For comfort, having the ability to customize the settings on the pump, like the suction strength and speed, is a very nice feature. The more you can mimic your baby’s feeding style, the easier your pumping sessions will be.

What’s included:

Look to see if there are any other pump parts that you need to purchase in addition to the breast pump. Or do they all come together in a kit?

Wearable:

Do you prefer to not have any tubing or bottles and be completely hands-free when pumping? Do you have a big family and need to save on time. Then a wearable pump might be the best breast pump for you.

Insurance:

Look to see which breast pumps will be covered by your health insurance policy. As of late 2012, private health insurers in the United States must cover breast pumps under the Affordable Care Act.

Cost:

I got to test the Willow pump out and I found it super convenient. At first, it can be a little confusing to get the right latch which is essential for a successful pumping session. However, their customer service is impeccable and helps you over the phone should you have any difficulties or questions. It also has no-spill technology and allows you to pump and store your milk in the same bag. This is great because it’s fewer steps for mom to do!

This double electric, battery-powered pump has a corresponding app that controls the suction (seven different levels) and keeps track of milk volume, pumping time, and previous pumping sessions. You can easily change the settings all on your phone. The battery also lasts all day.

Unfortunately, the Willow breast pump only works with breast cup sizes A-H. Also, because of the size of the Willow cups, the storage bags are only able to hold a smaller amount of breast milk than other pump containers. If you are a big milk producer, you may have to switch storage bags mid-pumping session. As for the price, it is more expensive. However, a portion of the fee may be covered by your insurance. An upgrade fee might be required depending on your insurance. But if your insurance doesn’t cover this pump, it is eligible for reimbursement under FSA/HSA. Also, the storage bags are also more expensive than standard breastmilk bags, and you can’t transfer milk from one bag to another nor can you reseal them.

This is why hospitals feel comfortable renting them out to multiple mothers. Whenever someone has a new baby, people love to celebrate them by bringing gifts. While stuffed animals and balloons are a sweet gesture, here are my favorite gifts to bring to a new family in the hospital that are a little more outside the box.

Best Gifts to Bring a New Family in the Hospital

Food

While food may not seem like a great gift for a new family, trust me, for a new mom, it is! After bringing a baby into this world and possibly not being allowed to eat for hours before that, new moms are hungry. I remember looking at the hospital menu with pure joy thinking I could and should eat anything I wanted.

Plus, her partner may not have had time to eat either and will appreciate the food too. My husband only had enough time to grab a few vending machine snacks. Most partners don’t pack a hospital bag, but maybe more of them should. After all, they might not have been in labor, but they will likely be at the hospital for the same amount of time as the mother.

Some good ideas are nuts, trail mix, cheese sticks, fruit, cookies, or something delicious like homemade zucchini bread!

Matching PJs

The first few days or weeks after having a baby, a new family usually wants to take it easy. So make this time extra cozy for them by bringing sweet matching PJ’s the whole family can wear as they learn about each other and (hopefully) get some rest. You might want to get the mom a nightdress/robe outfit instead of pants if possible. These would be easier to use at the hospital and even back home for a while.

Plus, if the family plans to get some newborn photography done, they can wear the PJs for a photo or two!

Gift Cards

While bringing a home-cooked meal during the first few weeks is always welcome, a nice gesture is to bring a few gift cards to the hospital. They are small to carry home, and the family can use them when necessary. You might want to ask what their favorite restaurants are first or bring gift cards that they can use for food delivery.

It can include practical items like large pads and painkillers, and also pretty socks, and a fresh-smelling candle. Here are some things to consider adding to your postpartum recovery kit and some other great recovery options if she had a C-section.

Flowers

Yes, flowers are pretty standard, but we still love them! You can help brighten their hospital room with a pretty bouquet they can take home with them. There is something about looking at a bouquet of flowers and feeling loved. You can either pick them up on your way over or buy a bouquet at the hospital gift shop on the way up to their room.

Your Photography Skills

If you’re a professional photographer, or just good at taking pictures, most new families would love to have those first few hours or days documented. This is a really sweet gift to bestow on a new family whose memory might be fuzzy thinking back to those first days but can remember them better with a couple of sweet pictures.

Just give mom a heads up first to see if she feels up for it. If she doesn’t feel well enough to do it at the hospital, you can always offer it for a few weeks later in the comfort of their own home.

Your Company

While a new family might need time to rest in the hospital, chances are they will love to see you if you are close to them. Just your support and care in coming to see them at the hospital says a lot.

As soon as my parents heard I was in labor, they began driving the eight hours to reach me the same day. My in-laws and a few friends from church stopped by as well. While the visits themselves can be sweet, they can also relieve dad of his duties for a bit if he needs to stretch his legs or run home to grab more clothes.

Just be sure to ask mom and dad if they want visitors because not everyone wants visitors at the hospital. And also, learn what to do and NOT to do when visiting a new baby.

Whether you plan on breastfeeding your baby exclusively or giving your baby a bottle from the beginning, we recommend every family to have a few baby bottles on hand when they welcome home their newborn. Some of the reasons are: that breastfeeding can be difficult. Sometimes it doesn’t go as planned. Or your child might have a milk allergy, and supplementing with formula might need to become an option. And the most common reason a breastfeeding baby may need to take a bottle is that sometimes mom may have to be away from her child before baby’s next feeding. Or mom might want her partner to help with feeding.

Because newborns typically eat every 2-4 hours, pumping allows mothers to build up an emergency freezer stash of breast milk if she ever needs a little break or has to be away before baby’s next feeding. But not all breastfed babies have the easiest time accepting a baby bottle when they’ve been breastfed. This is why it’s helpful to have bottles that have been successful with breastfed babies.

Do certain bottles really make a difference?

Yes and no. As a postpartum doula who has worked with hundreds of families, I’ve found that it really comes down to your baby and their specific needs and preferences. Some babies will accept the first bottle you give them. Others will be very picky and will down-right refuse the bottle until you give them one that they deem acceptable. But since there are tons of different bottles on the market, there are certain things that you can do with your baby and look for in a bottle to have success. Finding the best bottles for breastfed babies can help that transition and make it easier for you and baby. This way, we hope to spare you from buying and trying 12 different baby bottles before finding the right one for your baby.

When to Offer Your Baby a Bottle

When is the right time to introduce a bottle to your breastfed baby, you ask? Most lactation consultants recommend waiting until breastfeeding is well established and baby is 4-6 weeks old. However, I’ve experienced some infants refusing the bottle at 6 weeks old. This is why I suggest offering baby a bottle between 2-4 weeks old.

How and How Often Should You Offer Your Baby a Bottle

Only offer your baby a bottle 3-4 times a week and no more than once a day. Keeping it familiar but not too frequent should keep baby comfortable with the artificial nipple while also maintaining a strong breastfeeding relationship. Offering a bottle more frequently can cause your baby to prefer the bottle over the breast. If this happens, offer the bottle less frequently until breastfeeding is back on track.

When offering the bottle to your breastfed baby, you should practice paced bottle feeding. This helps the parent mimic the flow and rhythm of breastfeeding.

What to Look for When Choosing a Bottle for Your Breastfed Baby

Whether you’re browsing online or through the aisles at a store, there are several things that you should look at before making your baby bottle selection. The most successful bottles for breastfed babies have some key features.

The Bottle Shape

Some babies will want a bottle that feels familiar to the breast. Because of this, a wide-neck bottle or a breast-like shaped bottle can be helpful with this transition since it is ergonomically similar to the breast.

The Nipple Type

If you look at the different bottle nipples, you will notice different sizes and shapes. The nipples that are most similar to mom’s nipples are what we encourage you to try first. This will increase the chances of baby not refusing the breast or the bottle when switching back and forth. We suggest trying a wide-neck nipple base since that most closely resembles a mother’s breast.

The Venting

Good venting is important in a baby bottle because it reduces the amount of air a baby swallows. The less air that goes inside baby’s tummy, the better. When they swallow too much air, it can cause gas pains and increase colic.

It was a 15-minute drive home from the hospital. But during both pregnancies, I spent days deciding what outfit would be just the right thing to introduce my daughters to the world. For many babies, the coming home outfit is the first “real” clothing they will put on. It is also likely to be shared across social media, celebrating the big moment of leaving the cocoon of the hospital and entering the real world. It makes sense then that many moms-to-be put a little extra thought and planning into the iconic outfit.

Of course, that doesn’t mean you have to. Bringing a baby home from the hospital is a joyous (albeit slightly terrifying) time for new parents. The outfit they are wearing will not make or break the experience. However, there are some key things to remember when planning your newborn’s coming home outfit. From comfort, weather, and practical advice, we share our top tips for choosing a newborn’s outfit and highlight a few of our favorites below.

Tips for Choosing Baby’s Coming Home Outfit

Whether you live just around the corner from your birth center or hospital or have a long drive ahead, most babies will be traveling home in an infant car seat. Babies should be dressed, keeping both comfort and weather in mind. Here are a few things to keep in mind:

Dressing for the Weather & Warmth

Your location or time of year when giving birth may impact outfit choices. Of course, you want to ensure your baby is warm enough in the cold and keep them from overheating in warmer temperatures. However, as a rule of thumb, most new babies should be dressed with warmth in mind. Babies have more difficulty regulating their body temperature and therefore rely on us to dress them appropriately.1

A long-sleeve footed pajama or layette set is a great choice for most newborns. A hat and socks (if not using footed pants) will help keep them warm without risking overheating. Be sure to bring a few different sizes of hats if you want one to match the outfit you pick. I’ve experienced having too large of a hat and a baby with a giant head that did not fit a newborn hat. Fortunately, I was lucky to have a second option for both. Most hospitals provide hats that would also work perfectly fine for the car ride home.

A Few Practical Tips for Dressing Your Newborn for Travel Home:

  • While your focus might be on the car ride home, remember to dress your baby in something that will make life easier once you get home. Fancy is cute until you have to undress and redress a fussy or sleepy newborn.
  • Frequent diaper changes mean you will have to undo snaps or deal with complicated articles of clothing continually. So make it easy on yourself and go with something simple!
  • Bring two outfits (in two different sizes) in case you end up with a bigger baby. I had a 10-pounder, and she did not fit into many newborn outfits!
  • A onesie might work fine. But as a new mom, you will absolutely notice and cringe at the car seat straps touching their sensitive little skin, so a light pair of pants or leggings might be a helpful addition.
  • When looking for material, go for something soft and not scratchy, like cotton or bamboo.
  • Pro Mom Tip: Go for footed pants instead of pants and socks. They are much easier to use, stay on, and keep track of!

My personal preference is an adorable long-sleeved footed onesie (or footie pajamas as they are often called). This provides just enough coverage on the skin to protect against sun, wind, or chill while being light enough to help them remain comfortable (or as comfortable as you can be as a 1-3 day-old baby strapped into a bucket seat). In addition, they are easy to change diapers in and can transition from car seat to arms to bassinet without issue.
